Submit organization and event info as well as a copy of your organization's W9 and 501(c)3 status. The donation request must be submitted four to six months prior to the event. To apply for Grants or Sponsorships your organization must send a proposal along with copies of your organizations most recent 990 form, W9, and 501(c)3 status.
